Photos of the Food Truck:
- Please include a photo of your mobile unit.
Valid Vehicle Registration:
- Drivable food trucks and food trailers must have a valid vehicle registration.
Copy of Automobile Insurance:
- A copy of the automobile insurance for the mobile food truck or main vehicle that will haul the food trailer.
Health Unit Permit:
- Please contact the Caddo Health Unit at (318) 676-5265 for additional information.
Shreveport Caddo Tax Registration Certificate:
- Please contact the Caddo Shreveport Sales and Use Tax Commission at (318) 865-3312 for additional information.
Louisiana Sales Tax Registration Certificate:
- Please contact the Louisiana Department of Revenue online at (855) 307-3893 (option #3).
Commercial General Liability Insurance:
- Coverage must be at least $500,000, or $1,000,000 if operating in public
rights-of-way—with the City of Shreveport named as an additional insured.
Hold Harmless Agreement:
- This form is included the application packet.
Home-Based Business / Certificate of Occupancy:

- Please contact the City of Shreveport- Department of Finance / Revenue Division at 318-673-5500 for more information.
Shreveport Fire Department Inspection:
- All food trucks and trailers must pass inspection by the Shreveport Fire Department – Fire Prevention Division. For questions regarding requirements, please call 318-673-6740.
